[companion-integrate] RTI - Update of SFWgd package

This is a RTI for the update of the SFWgd package to version 2.0.34.

The webrev is at

The code has been reviewed and approved by Paul Cunningham.

The details include

1. Update of the source code.
2. Update of Makefile.sfw and install-sfw version number lines
3. Addition of gdfx.h file to /opt/sfw/include (needed in SFWgplot for 
example)
4. A patch is needed to change a reference to gd.h in one of the source 
files.
5. Copyright and other minor edits.
6. A METADATA file has been added.

This builds cleanly in a nightly on NV 62. Programs that use GD also 
build properly.
