Could be bug 6513165, "Error 25: Disk read error", when booting from usb dvd device
  http://bugs.opensolaris.org/bugdatabase/view_bug.do?bug_id=6513165

Root cause for bug 6513165 is a BIOS bug: the BIOS refuses to read from
the optical media in the usb dvd device after grub has loaded.  A BIOS
upgrade has fixed the problem for me.

At the grub prompt, try

    cat /<TAB>

That is, use grub's filename completion feature (press
the <TAB> key) to list the contents of the / root directory.
