It seems you are using early version of nevada(5.11, build 44 less).
If it is, you need to use ipw v0.3 which is gldv2 version. Please
download the package and install again.

If you have driver binary, a quicker way is to install it by:
#add_drv -i '"pci8086,1043"' ipw
