Well I'm definetly not running under Windows ;-)
The problem does not seem to have to do with file names, rather some change at some time.
I notice for changes in files we get "Remove stale file" followed by "Adding". For the files in question we just see "Remove stale file" (the files still exist). It looks like it has something to do with modification time because a touch of the file fixes the problem.
In cases of changed files I have observed the "Remove stale file" message repeated two or three times subsequently on the same file before "Adding".
