The most visible changes are
 - /etc/sudoers parsing is extended with an #include facility
 - a new option (-g) to specify a primary group for the command to run as
 - a new option (-A) to use a graphical prompter program
 - a new option (-n) to not ask for a password but exit with error if
   authentication is required
