Did you include a dropdown box with an option for Proxima Centauri ? 
At the very least, mars?  I mean hey, these are our nearest neighbors
and I am sure that they have a form posted with info for us located
right next door and if we can't be bothered to check then why should
they care about our registration forms?

Imagine the indignation upon seeing the registration forms on this
mostly harmless little blue planet.
