The snoop capture file is somewhat confusing.
It shows two different hosts as initiator!
As a double check, please would you capture another snoop file,
and post it back to this forum.
Make sure to use the 'tcp port 3260' filter when running snoop.
Try to capture both the 'discovery' session and
the 'normal' session.
Also make sure the period of the capture covers the
time when a core dump occurs, as recorded in
the iscsi target log file:
/var/svc/log/system-iscsitgt\:default.log
There is another DTrace script - 'iscsisnoop.d' - that may assist.

This DTrace script will list various iscsi target 'events'
